Essential duties and responsibilities include, but are not limited to:

* Implementing and coordinating the patient plan or car in a safe and organized manner.
* Providing ongoing evaluation of the patient's progress toward attainment of outcomes.
* Collaborating with the patient, family and others deemed appropriate in providing for continuity of patient care.
* Recognizing potential problems or crises and exercising appropriate nursing judgment and problem solving in a timely manner.
* Conducting lab services as directed.
* Providing services under the written order of the physician and as designated by the individual treatment plan.
* Accurately prepare the medication administration record for each patient and daily monitoring and update as required due to medication changes.
* Reviewing and monitoring patients physical and medical needs and services.
* Providing all required documentation including, but not limited to, written assessments, progress summaries, and input into individual treatment plans.
* Completing daily vitals for all patients.
* Meeting with patients daily to ensure all medical needs are being addressed.

Candidates must have completed an Accredited LPN Program and have a valid West Virginia LPN license.
