Title of Immediate Supervisor: Behavioral Health Nursing Supervisor
Position Description:
This non-exempt (FLSA) position's incumbent provides nursing services to individuals who are diagnosed with a behavioral health, substance use disorder and/or intellectual disability who receive psychiatric services through SBH behavioral health outpatient services. Obtains vital signs on individuals at the time of their medication evaluation and management appointments; provides individual and family education on Body Mass Index (BMI) and vital signs; assists psychiatric medication providers during appointments and as needed. Administers injections as prescribed by SBH psychiatric medication providers; performs venipunctures as ordered by psychiatric medication providers; monitors lab results and notifies psychiatric medication providers of any changes; obtains samples and prepares kits for pharmacogenetic testing. Monitors individuals for any adverse reactions to medications and informs the psychiatric provider. Maintains required documentation and medical records; reviews individuals' medical profiles in EHR and reconciles medication list; completes prior authorizations; coordinates with psychiatric providers to provide medication refills when needed. Participates in interdisciplinary staff meetings and coordinates with other SBH staff; coordinates with other medical providers such as primary care physicians.; assists with telehealth appointments as necessary.
Essential Functions (Tasks & Duties):
1. Performs nursing assessments and intakes as required and updates medication lists in the EHR. 2. Obtains blood pressure, height, weight and BMI of individual at the time of each psychiatric provider appointment, and documents in the individual's medical profile in the EHR. Provides individual and family education on Body Mass Index (BMI) and abnormal vital signs; assesses and monitors for possible onset of medical issues, such as metabolic issues; and refers individuals with elevated BMI and metabolic issues to primary care physicians and other medical providers, under the direction of the psychiatric provider.
3. Assists psychiatric providers during appointments and as needed.
4. Administers (I.M.) injections as prescribed by SBH psychiatric medication providers; and performs venipunctures as ordered by psychiatric medication providers; monitors lab results and notifies psychiatric medication providers of any changes.
5. Coordinates with and refers to primary care physicians for medical issues, labs, etc. and obtains medical records for psychiatrist to review.
6. Obtains samples and prepares kits for pharmacogenetic testing
7. Monitors individuals for any adverse reactions to medications and informs the psychiatric provider.
8. Maintains required documentation and medical records; reviews individuals' medical profiles in EHR and reconciles medication list each visit.
9. Coordinates with psychiatric providers to provide medication refills when needed; maintains prior authorizations for individual's medications as required by insurance.
10. Participates in interdisciplinary staff meetings and coordinates with other SBH staff; coordinates with other medical providers such as primary care physicians.
11. Monitors client appointments and appropriate follow-up. Reviews psychiatric providers schedules and coordinates with Centralized Scheduling, the Lead Therapist and Lead CM to fill the schedule.
12. Assists with telehealth appointments as necessary
13. Provides nursing coverage in other agency locations as needed.
14. Performs other duties as assigned by the supervisor consistent with SBH policies and procedures.
Part IV - Position Qualifications
The entry level qualifications for this position are as follows:
A. Requisite knowledge, skills and abilities:
1. Knowledge of psychiatric and general nursing skills.
2. Knowledge of mental health and substance use disorders in the adult, child and adolescent population.
3. Knowledge of psychotropic and limited primary care medications and their adverse reactions.
4. Knowledge of medication record keeping as required by regulations.
5. Knowledge of appropriate disposal of medications and medical waste.
6. Knowledge of medical conditions, community resources and service coordination. 7. Ability to document accurately.
8. Ability to work with diverse individuals.
9. Ability to communicate clearly and effectively (verbally, interpersonally and in writing).
10. Ability to maintain a high degree of confidentiality.
B. Special licenses, registration or certification: Licensed Practical Nurse (LPN)-VA or Registered Nurse (RN)-VA; current CPR and First Aid certifications; Controlled Substance Registration; valid driver's license.
C. Education or training (cite major area of study): Licensed Practical Nurse formal training.
D. Level and type of previous work experience: Two or three years of nursing experience required; one to two years of providing services to individuals with mental health and/or substance use disorder preferred.
