- Assess and evaluate patient needs/problems and develop patient care plans
- Provide speech therapy services including evaluation, teaching, and training activities
- Report patient status to Clinical Supervisor and referring Physician
- Document changes to patient care plans and assist with discharge planning
- Attend staff meetings, conferences, and educational in-services
- Perform OASIS time point assessment and submit recertification paperwork
Qualifications
- State Professional License and current driver's license with insurance
- One-year experience as a home care Speech Therapist
- Ability to make sound clinical judgment and formulate individualized care plans
- Proficiency in clinical skills and Microsoft Office
