Job Description:
This position is responsible for coordinating patient care transitions and providing social work services to patients and their families in a hospital setting. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
Key Responsibilities:
Conduct comprehensive psychosocial assessments to identify patients' health goals and coordinate services for effective transition planning.
Develop, implement, and revise care plans to ensure good transition management, continuity of care, cost reduction, and patient self-management.
Collaborate with physicians and multidisciplinary teams to facilitate care and meet discharge goals.
Provide education and support to patients and their families to help them cope with hospitalization and post-acute care needs.
Requirements and Qualifications:
To be successful in this role, you will need:
A Bachelor's degree in Social Work (BSW) from an accredited school of social work.
A minimum of 2 years of clinical experience as a social worker in a medical/clinical setting.
Excellent interpersonal communication and negotiation skills, with the ability to work with people of all social, economic, and cultural backgrounds.
Computer skills and proficiency in relevant software applications.
Preferred certifications include CCM and ACM.
