Job Description:

This position is responsible for coordinating patient care transitions and providing social work services to patients and their families in a hospital setting. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:

  • Conduct comprehensive psychosocial assessments to identify patients' health goals and coordinate services for effective transition planning.

  • Develop, implement, and revise care plans to ensure good transition management, continuity of care, cost reduction, and patient self-management.

  • Collaborate with physicians and multidisciplinary teams to facilitate care and meet discharge goals.

  • Provide education and support to patients and their families to help them cope with hospitalization and post-acute care needs.

Requirements and Qualifications:

To be successful in this role, you will need:

  • A Bachelor's degree in Social Work (BSW) from an accredited school of social work.

  • A minimum of 2 years of clinical experience as a social worker in a medical/clinical setting.

  • Excellent interpersonal communication and negotiation skills, with the ability to work with people of all social, economic, and cultural backgrounds.

  • Computer skills and proficiency in relevant software applications.

  • Preferred certifications include CCM and ACM.
