Complex Care Community Health Worker

POSITION SUMMARY:
Support medically complex individuals diagnosed with severe and persistent mental illnesses in accessing needed medical care.
This is a four-year grant-funded position created to provide additional support to individuals enrolle d in the Integrated Health Home and experiencing an exacerbation of their medical condition ( s ) . The ideal candidate has an interest in assisting their clients to improve their management of specific health challenges . Working with an experienced RN, t he C omplex Care Case Manager provides support for medical appointment attendance , medication adherence, access to community resources, and home environment improvement. This is an opportunity to learn about a variety of medical conditions and their optimal management while you participate in impr oving the lives o f your clients. You must drive a safe, accessible, registered and insured vehicle and be willing to transport clients.
This is a full-time position perfect for a compassionate, organized and motivated applicant considering a career in nursing , OT/ PT or another healthcare field . Some scheduling flexibility may be available to candidates enrolled in such programs. Previous experience as a Community Health Worker, EMT, or CNA will be considered in place of an associate or bachelor's Degree in a health or human services field.
