**Physical Therapist (Temp to Perm Opportunity)**

Phaxis, a premier travel healthcare staffing agency, is seeking a dedicated and experienced Physical Therapist for a temp to perm opportunity in the vibrant city of Jacksonville, FL. This role begins with a 13-week contract, with the potential for a permanent position upon successful completion of the assignment.

**Position Overview:**

As a Physical Therapist, you will provide high-quality rehabilitation services to patients, helping them improve their physical function and mobility. You will work in a supportive and dynamic healthcare environment, offering individualized care and making a significant impact on patient outcomes.

**Key Responsibilities:**

- Develop and implement personalized treatment plans for patients
- Conduct thorough assessments and evaluations of patients' physical conditions
- Use a variety of techniques and therapies to assist patients in their rehabilitation
- Monitor and document patient progress, adjusting treatment plans as necessary
- Collaborate with a multidisciplinary team to ensure comprehensive patient care
- Educate patients and their families on exercises and activities to aid in recovery

**Requirements:**

- **Experience:** Minimum of 2 years of experience as a Physical Therapist
- **Licensing:** Must be licensed to practice as a Physical Therapist in the state of Florida
- **Skills:** Strong clinical skills, attention to detail, and excellent communication abilities
