POSITION SUMMARY:

With general direction from the Hospice Physician (or Nurse Practitioner) and under the supervision of a Registered Nurse (RN), the Licensed Vocational Nurse (LVN) provides direct nursing care and related assistance to patients and families as outlined in the hospice plan of care. The LVN utilizes monitoring, intervention and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit. Although the LVN reports to a primary manager, HEB expects the LVN may work in more than one program.

ESSENTIAL FUNCTIONS:

  • Provides direct nursing care to terminally ill patients in the Inpatient Unit under the direct supervision of an RN.
  • Administers medications as prescribed by the MD/NP.
  • Evaluates patient /family needs, the effectiveness of treatments and monitors patient's reactions.
  • Collaborates with the HEB interdisciplinary team in the development of the individualized patient plan of care
  • Supports, instructs and educates patient, family and caregiver.
  • Communicates effectively with patients, families, and team members.
  • Maintains the dignity, confidentiality and privacy of the patient.
  • Completes documentation timely and accurately.
  • Participates in the agency quality improvement activities as assigned.
  • Adheres to and complies with state and federal regulations, as well as accreditation standards.
  • Performs other duties as assigned.
POSITION REQUIREMENTS:
  • Demonstrates the skills, knowledge and abilities necessary to be successful in the job as outlined in the position requirements.
  • Must have excellent interpersonal, communication and organizational skills, knowledge of pain and symptom management, knowledge of hospice regulations.
  • Ability to establish and maintain cooperative relationships with professionals and families.
  • Ability to collaboratively work as part of an interdisciplinary group.
  • Willingness and flexibility to be cross-trained in more than one HEB program or department.
EXPERIENCE AND EDUCATION:
  • Current, active LVN license required; must be verified by the CA Board of Vocational Nursing and Psychiatric Technicians.
  • At least one year nursing experience as LVN required; experience working in acute care and/or with end-of-life patients highly desirable.
  • Current CA Driver's License and car in good working condition required.
  • Current CPR certification required.
  • IV certification preferred.
