Job Summary:
Reports to the Practice Manager, the Dietitian is responsible for providing nutritional services to individuals in an outpatient setting for nutrition-related problems, preventative health measures, and chronic disease management. Primary responsibilities include assessing and monitoring the nutritional health of ambulatory patients; developing and implementing individualized nutrition care plans; providing nutrition counseling and education for patients, families, and groups; collaborating with the multidisciplinary team to facilitate optimal nutritional outcome; maintaining professional competency and skills required for professional practice.

DUTIES AND RESPONSIBILITIES:

1. Interview patients to obtain nutrition assessment data and information regarding usual dietary patterns, lifestyles, food purchasing practices and resources, and the ability to understand/ comply with diet changes. Evaluate and assess nutritional status in accordance with established policies and evidence-based standards; age-specific populations; and culture, religious, and ethnic concerns. Develop and implement an individualized plan for nutrition intervention in accordance with patient's medical program goals, nutrition prescription, and self-management training. Assist patients in developing realistic, long-term health goals and goal-directed behaviors. Communicate assessment of nutrition status and education to other health care providers via documentation in patient's medical record. Counsel patients to ensure dietary compliance, nutritional adequacy, and formulation of heath goals.

2. Assess and adjust diet regimen based on weight changes, lab values, clinical parameters, patient compliance and understanding. Evaluates the effectiveness of Medical Nutrition Therapy and measures progress towards the outcome of the Medical Nutrition Therapy interventions.

3. Education of Patients/families; plans, evaluates, and provides individual and group education to patients, family, and/or significant other in diet and lifestyle changes, nutritional expectations, and behavior modification based on assessment of individual learning needs; develops/reviews education materials to be utilized in patient education. Collaborates with physicians and other team members to ensure patient education needs are met.

4. Assists in the development of nutrition related standards of practice within the Health System. Facilitates nutrition care planning throughout the duration of care. Work as a team in program planning, curriculum and department goal development. Develops and updates nutrition education materials for inpatient and outpatient arenas. Interprets, evaluates, and utilizes pertinent research in disease management to assist in best practice outcomes for the patient. Evaluates new products based on their patient appropriateness and safety. Manages and requests appropriate educational resources. Assist and support the manager in maintaining program requirements.

5. Maintain active registration and licensure. Maintain high level of competency and professional development through available resources in advanced bariatric nutrition to ensure the best surgical and long-term weight loss outcome. Participate in departmental and interdisciplinary meetings, task forces, and projects.

6. Participate in research protocols and devilment of clinical pathways in service, research protocol and development of clinical pathways in assigned areas of responsibility. Attends required staff meetings and completes mandatory in-service/requirements.

7. Provide supervised learning experiences for dietetic interns when appropriate. Serve as a resource and lecturer for healthcare providers, students, and the general public to support the organizations initiative for the community. Participate in the orientation and training of new dietitians as directed.

8. Enforces applicable safety and infection control procedures to ensure the safety of patients, families, and all other associates. Complies with Medical Center, state, JCAHO, and federal safety standards.

9. OSHA, CLIA and HIPAA compliance.

10. Coordination and communication of evidence-based nutritional recommendations for micro/macro nutrients, laboratory, parameters and diet standards to physicians, nursing staff, and clinical dietitians. Analyze micronutrient data, recognize signs and symptoms of deficiencies and communicate recommendations to surgeon.

11. Maintains necessary CEU requirement for licensure/ certification.

12. Other duties as assigned.

REQUIRED SKILLS AND KNOWLEDGE:

Ability to manage and prioritize multiple tasks, knowledge of Excel, Word, Outlook and Powerpoint and the ability to learn other computer skills. Must have good organizational skills and work professionally with doctors, hospital administration and management, SEP employees and the public.

OTHER REQUIRED SKILLS AND KNOWLEDGE:

Comprehensive knowledge and application of medical nutrition therapy. Knowledge of human behavior and surgical weight loss techniques for effecting behavior change. Ability to communicate effectively in both written and oral form to patients, public, medical staff, and physicians. Good interpersonal skills, problem solving, decision-making, time management and leadership skills. Ability to counsel and motivate others, as individuals and as a group. Development and implementation of effective nutrition education plans/programs for a variety of target of target audiences.

LICENSES AND CERTIFICATIONS:

Minimum:

• Current registration to practice dietetics in the state of Kentucky.

• CDCES Certification (Certified Diabetes Care and Education Specialist) previously CDE (Certified Diabetes Educator).

EDUCATION:

Minimum: Graduate of accredited university or college and course work accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND). Current registration to practice dietetics in the state of Kentucky. Certification as a Diabetes Care and Education Specialist.

YEARS OF EXPERIENCE:

Minimum: Three years' experience in clinical dietetics.
