The Direct Service Provider (DSP) delivers basic care to developmentally disabled adults in the individual's own private residence. Care is provided in accordance with the Personal Plan of Service which is different for each person being served depending on their own unique needs. These needs could include but are not limited to personal care, transportation, teaching, guiding, and supporting individuals in meeting their own personal needs, dreams, and desires.

RESPONSIBILITIES:

  1. Perform all assignments in accordance with established organizational policies and procedures as outlined in the organizations' policy manual, employee handbook, and as instructed by the supervisor.
  2. Provide direct care according to the individual's plan of care (ISP).
  3. Documenting care and medication according to company and state policy and procedures.
  4. Input clinical data into Home Health Software as instructed.
  5. Meet with the family to discuss the needs of patients.
  6. Teach developmentally disabled adults how to adopt basic independent living skills.
  7. Help physically disabled move from room to room around their homes.
  8. Assist individuals in dressing, bathing, and completing basic hygiene tasks.
  9. Oversee any required rehabilitation exercises.
  10. Ensure that individuals take their prescribed medications or administer medications: paid training provided.
  11. Prepare meals that adhere to special dietary considerations.
  12. Help adults with developmental disorders learn to make a budget and complete basic household tasks.
  13. Assist/complete home keeping chores: Dishes, laundry, vacuum, mopping, ETC.
  14. Transportation to and from work/programs ( M-F as needed), transportation for social and medical activities/appointments.
  15. Runs errands to the grocery store, laundromat, cleaners, drugstore, and other necessary locations.
  16. Serves as a companion. Organizes activities for patients.
  17. Perform all job duties in a manner that is friendly, respectful, and aligned with the agency's Mission, Values, Patient Care Philosophy and Standards of Ethical Conduct.
  18. All other duties as assigned.
QUALIFICATIONS
  1. High School Diploma or equivalent.
  2. 2+ experience providing direct care service to adults with a variety of disabilities.
  3. Must be 18 years of age or older.
  4. Must be certified within 60 days of starting the position on First Aid/CPR.
  5. Maintain a current and valid Ohio or Michigan driver's license, a good driving record, adequate insurance, and be willing to use personal car to perform job duties.
  6. Basic computer skills.
  7. Strong organizational, time management, multi-tasking, and problem-solving
  8. Be able to pass a DRUG TEST and BACKGROUND CHECK.
PHYSICAL REQUIREMENTS:
  1. Standing: Maintaining an upright position on the feet, especially for prolonged periods of time
  2. Pushing: Applying pressure and exerting force against an object for the purpose of moving it forward or away from the source of the force - push or pull force up to 35 lbs.
  3. Lifting: Raising an object from a lower to a higher position, or moving an object laterally, from position to position-Required to lift up to 50 lbs of force independently. May be required to lift over 50 lbs with assistance of another person or a mechanical lift.
  4. Stooping: Bending the body forward and down from the waist, hips, and knees, sometimes for a prolonged length of time.
  5. Reaching: Extending hand(s) and arm(s) in any direction away from the body
  6. Balancing: Maintaining body equilibrium to prevent falling when walking, standing, or crouching on narrow, slippery, or moving surfaces
  7. Ability to safely operate a personal motor vehicle on a weekly basis and comply with the Company's Vehicle Safety Program.
