Position: Licensed Practical Nurse -

Location: Greenville, OH, US -

Number of Positions: 2

LPN - Full-Time Position:- Hours: Monday to Friday, 8:00 am to 4:30 pm - Coverage Area: Primarily Mercer and northern service area - Rotation: Weekends and holidays

LPN - Full-Time Position:- Hours: Monday to Friday, 12:00 pm to 8:00 pm - Rotation: Weekends and holidays

Responsibilities

• Make visits to the patient's home, whether that be a personal residence, SNF, AL - anywhere they call home to provide hospice services.
• Answer referral calls and inquiry questions about hospice, hospice benefits, and palliative care.
• Obtain orders from the Medical Director and Attending Practitioners.
• Assist with paperwork, including physician orders and data entry into the EMR.
• ordering medication, DME, and setting up transportation for patients.
• Coordinate the admission with the discharge planner at the hospital when applicable.
• Organize all obtained information and enter the required data into the EMR.
• Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
• Prepare and complete paperwork according to Agency requirements.
• Audit new admissions for accuracy.
• Follow up with patients in the In Touch Program.
• Assist with pre-admission activities, including but not limited to referrals, ordering medications, ordering DME, and setting up transport when needed.
• Complete referrals and pre-admit contact.
• Obtain patient health information and enter required data into the EMR.
• Assist with data entry, including but not limited to physician orders, frequency orders, medication, and DME.
• Assist with QAPI audits.
• Oversee the admission binder process.
• Other duties as assigned.

Qualifications

• Graduation from an approved School of Practical Nursing.
• Current LPN licensure in the State of Ohio.
• Minimum of one year of experience in Medical/Surgical, Oncology, or community health nursing.
• Hospice experience preferred.
• Shall have a valid driver's license.
• Excellent customer service skills.
• Detail-orientated with excellent communication, documentation, and organizational skills.
• Ability to work independently without supervision.
• Travel to other sites as needed. Travel may be in inclement weather.
• Ability to lift 25 lbs. of supplies or equipment.
• Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
• Weekend and Holiday rotation.
