License: State license in Physical Therapy required; Compact License acceptable if in a participating state

Certifications: Basic Life Support (BLS) from AHA or ARC required

Must-Have:
- Bachelor's Degree in Physical Therapy program

Job Description:

- Evaluate, plan, direct, and administer programs of Physical Therapy.
- Conduct Physical Therapy programs to restore function, prevent disability, and help patients enhance physical and functional abilities.
- Evaluate patients, select and administer assessment techniques, synthesize information to establish appropriate patient care plans.
- Develop necessary modalities, procedures, exercise programs, therapeutic groups, and patient education based on a holistic view of the patient's diagnosis, condition, age, and discharge needs.
- Gather and interpret data from medical records, physician reports, patients, and families.
- Establish functional goals with patients and provide evidence-based interventions to achieve these goals.
- Adjust plan of care as needed based on ongoing assessments.
- Document evaluations, progress notes, treatment notes, education, outcomes, discharge notes, and charges.
- Communicate evaluations and progress to physicians, referral sources, and other clinical staff.
- Coordinate patient schedules, treatment programs, and discharge planning.
- Supervise Physical Therapist Assistants and Rehab Tech/Aides as required.

Skills:

- Proficiency in patient care, discharge planning, and documentation.
- Physical ability to demonstrate exercises and activities.
- Ability to work in a fast-paced clinical environment.
- Required physical activities include sitting, standing, walking, climbing stairs, kneeling, twisting, bending, crouching, crawling, and reaching overhead.
- Ability to lift patients and equipment according to specified requirements.
