The purpose of Co-Responder Outreach Services is to provide the Department of Social Services and Law Enforcement Agencies with access to a Co-Responder Outreach Specialist who connects individuals with behavioral health needs including mental health, substance use, and intellectual developmental disabilities to available services and resources in their community. The Co-Responder Outreach Specialist will respond with officers and Department of Social Services’ staff to calls involving behavioral health issues when they occur and/or provide outreach to individuals after a behavioral health crisis. This will involve coordination with the Mobile Crisis Management Services Provider who may initially respond to the behavioral health crisis. Co-Responders will perform outreach to individuals who are at risk of entering or who have entered the criminal justice system as well as those served by the Department of Social Services. Co-Responders will (assess the social determinants of health needs), complete a case management assessment, develop a service plan, and provide time limited case management to address the identified social determinant of health needs.

QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIREMENTS

Must be credentialed as a Qualified Professional. A Qualified Professional (QP) is an individual who is a) a graduate of a four-year college or university with an advanced degree in a human services related field with documentation of at least one year of supervised experience; b) a graduate of a college or university with a bachelor’s degree in a human services related field with documentation of at least two years of supervised experience; c) graduate of a college or university with a bachelor’s degree in a field other than human services with documentation of at least four years of supervised experience.

A Minimum of 2 years’ experience with serving individuals who have behavioral health issues. A minimum of 1-year experience with serving individuals in crisis.
