Job Description

POSITION IS ELIGABLE FOR HYBRID-REMOTE WORK AFTER 90 DAY PROBATION/TRAINING PERIOD IN THE ALBUQUERQUE METRO AREA.

JOB CATEGORY: CARE COORDINATION / COORDINATION / CASE MANAGEMENT

POSITION SUMMARY: Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, etc.) by proactively coordinating care for patients to facilitate the achievement of quality and cost effective health outcomes.

Qualifications

REQUIREMENTS

EDUCATION AND/OR EXPERIENCE:
  • Associate's degree AND five year's relevant healthcare experience; OR
  • Bachelor's degree AND four year's relevant healthcare experience; OR
  • Master's degree AND two year's relevant healthcare experience. Self-starter with strong verbal and written communication skills.
  • Comfort with change and quality improvement processes.
  • Experience with Electronic Health Records strongly preferred.
  • Experience with Motivational Interviewing strongly preferred.
  • Knowledge of New Mexico and local community resources strongly preferred.
  • Bilingual (English / Spanish) or (English / Navajo) preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
  • Certified Case Manager (CCM) preferred.
SCREENING REQUIREMENTS:

This position requires successful completion of an initial post-offer of employment:
  • Criminal Records Check.
  • According to PMS Human Resources Policy 205.006, "Healthcare Personnel Testing & Immunization Requirements," this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee's PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH.
