Reports to the Home Care Manager. Establishes Home health services in the patient's home. Evaluates plans and administers a treatment program designed to assist physically or psychosocially disabled patients to achieve their highest level of cognitive, perceptual and motor functioning. Coordinates visit frequencies and patient plan of care with certified Occupational Therapy Assistant.

Evaluates patients to determine present and potential level of occupational performance by:
  • Testing psychological/interpersonal skills, cognitive skills, motor skills and sensory integration.
  • Administering and interpreting both standardized and non-standardized tests, reviewing patient's medical and psychosocial history, and protocols pertinent to the particular patient.
Establishes therapy goals and formulates a treatment plan consistent with goals and based on evaluation by:
  • Analyzing data about the patient to determine, design, and implement therapeutic motor activities that promote or conserve optimal physical function within patient disability limits.
  • Discussing evaluation/treatment/patient goals with patient/family/caregiver.
Obtains cooperation and continuity in therapy by:
    • Discussing evaluation/treatment/patient goals with patient/family/caregiver.
    • Participating in family conferences.
    • Participating in patient care conferences.
Maintains records and charts of patients per established schedule by:
  • Documenting treatment provided and progress of the patient in compliance with State, Federal, JCAHO and other regulatory agency requirements.
  • Informing the physician of changes in the patient's condition.
Educates other health care personnel including home health aides and patient families/caregivers in routine phases of occupational therapy by:
  • Participating in family conferences.
  • Presents annual in-service/training for rehab staff, nursing, home health aides and/or community organizations.
Maintains professional knowledge and competence in areas of responsibility by:
  • Attending in-services and department meetings; also participating in continuing education and attending conference as approved.
  • Maintaining professional affiliations as appropriate.
  • Reading current professional literature and journals.
Contributes to the overall effectiveness and efficiency of the Department by:
  • Completing other job-related duties and projects as assigned.
ORGANIZATIONAL RESPONSIBILITIES

Associate complies with the following organizational requirements:
  • Attends and participates in department meetings and is accountable for all information shared.
  • Completes mandatory education, annual competencies and department specific education within established timeframes.
  • Completes annual employee health requirements within established timeframes.
  • Maintains license/certification, registration in good standing throughout fiscal year.
  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.
  • Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience

The above level of knowledge, skills and abilities are normally acquired by the completion of a bachelor's degree (Master's prepared Degree) program in Occupational Therapy, validated by the maintenance of licensure to practice as an Occupational Therapist in the state of Michigan and in the state in which the employee practices as an Occupational Therapist. Two to three years of clinical experience is required. One year of home care experience is preferred.

Knowledge & Skills
  • Requires thorough knowledge of physiology, psychology and the application of various treatment modalities for the enhancement or restoration of cognitive, perceptual and motor functioning.
  • Requires the analytical and organizational skills necessary to assess patient's condition, develops effective treatment programs, and evaluate the patient's progress.
  • Demonstrates the interpersonal and communication skills (both verbal and written) necessary to articulate ideas clearly and concisely, to make effective presentations, to develop and maintain positive working relationships, to interact effectively with a wide range of internal and external contacts in a variety of situations, to report appropriate information needed by physicians, home health care members, and third-party payors; and to instruct patients, families, and groups.

  • Requires a valid driver's license, access to a vehicle and proof of automobile liability insurance coverage.
Working Conditions
  • Works in an office environment and patients' place of residence.
  • Travel via vehicle is required to visit patients in their place of residence.
  • Exposure to patient care activities.
  • Possible exposure to biohazards.
Physical Demands
  • Requires the physical ability and stamina (i.e., to lift and position patients, perform treatment modalities, etc.) to perform the essential functions of the position.
