Acenda Integrated Health is recruiting for a Case Manager to support the Project in Assistance in Transition from Homelessness (PATH) program in Cape May, New Jersey.

Program Overview:

Project in Assistance in Transition from Homelessness (PATH) is an intensive case management service that provides mobile outreach services to the psychiatrically disabled homeless living in the community. Services provided to individuals suffering from serious mental illness, or suffering from serious mental illness and from substance abuse, who are homeless or at imminent risk of becoming homeless. The program implements the concepts of homeless outreach services and assists with placement into emergency and other appropriate housing. Case maintenance services include those related to activities of daily living, personal financial planning, transportation services, habilitation and rehabilitation, prevocational and vocational services, and housing services.

Key Responsibilities:
  • Works in collaboration and continuous partnership with clients, community services, and providers to ensure the client's care plan
  • Effectively implements and adequately addresses the client's needs
  • Connect clients to relevant community resources including substance abuse treatment, housing, employment, mental health services and primary care.
Requirements:
  • Bachelor's degree in psychology, social work or a related field
  • 2 years of experience of relevant experience
  • Knowledge and/or experience in trauma-informed care, mental health and substance abuse.
  • Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record.
Additional Information:
