Description

The ACT Therapist / Evergreen Liaison is an integral part of a multi-disciplinary team of mental health providers who provide quality mental health treatment services to persons receiving ACT services and/or are residents of Crosswinds group home, Evergreen. The ACT Therapist / Evergreen Liaison works under the direction and supervision of the ACT Manager, providing Evidence Based Practice interventions in individual and group therapy sessions and within the therapeutic milieu setting of Evergreen.

Essential Functions
  • Works with a multidisciplinary team, (including psychiatry, nursing, therapy, case managers, peer support, residential specialists, and attendant care workers), attends and contributes to ACT and Evergreen staff meetings.
  • Builds trusting and professional relationships with clients by participating with clients and vested individuals in a dependable and purposeful manner on a routine basis. Serves as an effective advocate for clients by coordinating with community agencies and within the organization to ensure delivery of necessary services.
  • Serves as an in-house clinical consultant for the Residential Services Manager, promoting a therapeutic milieu by providing recommendations, guidance for Evergreen staff and residents and overseeing the behavior focused point/level program. Assists in meeting the designed goals of Evergreen as a group home residence to prepare adults with severe and persistent mental illness to successfully live independently.
  • Establishes and maintains a positive working relationship and professional demeanor with clients and families, external community partners, Agency staff, and visitors.
  • Provides direct family, individual, and group therapy services to adult clients that are part of the ACT program and/or are residents of Evergreen.
  • Conducts clinical intakes, evaluations, develops and updates individualized treatment plans with goals and objectives, discharges and other related documentation as needed.
  • Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, meeting Crosswinds' standards and additional requirements.
  • Completes reviews and maintains proper documentation of client treatment records including intakes, diagnosis, referrals for ACT, treatment plans, crisis plans, DLA, Evergreen point cards and other documentation related to intakes, assessments, and 90-day reviews.
  • Works in partnership with clients and other vested individuals to design and implement a comprehensive and individualized plan of care based on a strengths assessment.

Work Environment

This job operates both in a professional office setting and in the community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Position Type/Expected Hours of Work

This is a full-time position: Monday Friday, minimum of 40 hours per week, with on-call and weekend coverage, as needed. Hours are flexible with the expectation that at least 4 hours per week will be outside of the hours of 8:00-5:00.

Travel

Ability to provide transportation to patients, as well as the ability to travel to deliver services and respond to urgent and emergent situations. This position shall primarily stay within the confines of Lyon County but may require occasional travel outside of Lyon County.

Qualifications

Required Education and Experience

Master's degree in psychology, social work, counseling, marriage and family or related field. At least one full year of experience working with SPMI population. Familiarity with community mental health services including medication clinic, case management, housing, IPS, SUD, peer support, attendant care, and SOAR.

Licensure/Certification

Licensure as a master or doctorate level clinician with the Kansas Behavioral Sciences Regulatory Board. LSCSW or LP licensure is preferred.

Additional Eligibility Qualifications

Ability to work independently and as a member of a therapeutic team; ability to work with persons with severe emotional disturbance and/or severe and persistent mental illness; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations; possess prior computer experience in Microsoft Office programs and basic computer fluency; maintain a driver's license and a driving record consistent with reasonable insurability (no alcohol-related incidents in the last five years).
