Highland Rivers provides treatment and recovery services to individuals impacted by mental illness; substance use disorders and intellectual developmental disabilities.

JOB SUMMARY:The Case Manager maintains a caseload of Intellectual Developmentally Disabled individuals. The Case Manager teaches and coaches the individuals and assists in acquiring, retaining, or improving socialization and networking, independent use of community resources, and adaptive skills required for active community participation outside the home.

Duties and Responsibilities include:
  • Assume responsibilities for Program Managers during their absence.
  • Monthly documentation (Summaries, HRST updates, Case Manager notes,
  • Oversees locked medication for individuals and return unused medication, maintain monthly MAR.
  • training for staff as indicated in individuals service plans /HRST.
  • Knowledge of DD services and funding sources
  • Communication/customer service (emails, phones calls, meetings)
  • Management, maintenance, and audits of electronic health records, including ECR page.
  • Document Review (PA, DMA6 and DMA7, ISP, HRST, SIS)
  • Complete trip orders for transportation
  • Complete discharge and paperwork
  • Crisis intervention, monitor and maintain health and safety of individuals.
  • Complete incident reports (non-critical, critical, death reports and follow ups)
  • Assist with instructor duties as needed (CAI, feeding, toileting, supervision, etc)
  • Transporting individuals into the community or when there is an emergency.
  • Scheduling staff
  • Maintain all training requirements.
  • Monitoring new staff training.
  • Health and Safety Officer at Day Program
  • Trips completed (if applicable) on time
  • Other duties as assigned.
Requirements

MINIMUMQUALIFICATIONS:
  • High School diploma or GED required.
  • Bachelor's Degree preferred
  • At least one year's experience working in mental health or substance use disorder field preferred.
  • Must be able to pass background investigation, drug screen and have good 7year motor vehicle report.
  • Bi-Lingual abilities a plus
PREFERRED QUALIFICATIONS:
  • High School Diploma or GED required.
  • At least 6 months experience with Developmental Disabilities preferred.
  • Computer skills, Organizational and teaching skills, ability to work with minimal supervision.
  • Must be able to pass background investigation, drug screen and have good 7yr. motor vehicle report.
  • Bi-lingual abilities a plus
