GENERAL POSITION SUMMARY:

IRCO Washington County Supportive Housing Services works to provide the highest quality of Housing Case Management services to participants currently experiencing homelessness and housing instability and who are eligible for supportive housing funded through the Metro Supportive Housing Program in Washington County. This position is both sites based, and field based. Retention Case Managers will have a maximum of 30:1 ratio of stable and housed households derived from the existing case management.

ESSENTIAL FUNCTIONS:

  • Provide a participant centered approach and excellent customer service that is sensitive to the challenges of homelessness, including medical and behavioral health issues that face them, in their efforts to successfully maintain permanent supportive housing.
  • Employ a "whatever it takes" approach to assist participants in keeping and maintaining their permanent housing.
  • Assist households through the stages of the housing stabilization process including flexible services to meet the needs of participants.
  • Working effectively with other supportive case managers in the transition process establishing new relationships with existing participants.
  • Establish a working and professional relationship with participants.
  • Conduct monthly home visits and regular check-ins with participants.
  • Provide connection to other providers and services when needed.
  • Post a minimum of 2 case notes per participant per month.
  • Ensure all documentation is effectively maintained in both internal and external systems.
  • Assist in all levels of maintaining permanent housing including working with property management and utility services.
  • Conduct assessments, goal setting, establishing time frames, in collaboration with the participant.
  • Ensure access to physical and mental health, including substance use disorder services. Assists participants with establishing a medical awareness home.
  • Ensure connection with medication and treatment regimens.
  • Assist with job search and placement, increasing income, education, and social services.
  • Assist with independent living skills, including social, personal hygiene, budgeting and money management, legal services, and transportation as needed.
  • Ensure participants understand tenant rights and responsibilities.
  • Ensure participants are aware of emergency services such as 911 and crisis intervention services.
  • Provide eviction counseling; work with property management to resolve issues that threaten housing stability.
SECONDARY FUNCTIONS:

Attend weekly check-in meetings, all staff meetings, funder meetings, case conferencing meetings as deemed necessary. Timely and responsive communication to coordinator/supervisor and other team members through text messaging, emails, group messaging, and phone calls.

Requirements

JOB SPECIFIC QUALIFICATIONS:
  • Possess an associate degree or higher in a human service or related field and minimum of 2 years of experience in providing related services to similar population.
  • Or possess a bachelor's degree in a human service or related field and minimum of 1 year of experience providing related services to similar populations.
  • Lived experience will be taken into consideration of educational requirements.
  • Knowledge or ability to learn the Homeless Management Information System (HMIS)
  • Experience working with participants with mental health issues, chronic health issues, and/or substance use disorders.
  • Experience in the following areas: chronic homelessness, outreach and engagement strategies, housing navigation, best practice models, mental health, substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing, and landlord/tenant rights/eviction prevention, etc.
  • Working knowledge of Microsoft Office products, including Word and Excel.
  • A valid Oregon driver's license, possession of reliable transportation and car insurance.
  • Must be organized and detailed oriented, ability to manage multiple cases efficiently and effectively.
  • A passion for ending homelessness.
  • Ability to work both independently and as a member of a team.
  • Possess strong people skills and work ethic.
  • Ability to work effectively and productively with people of varying races, ethnic, cultural, educational, and socio-economic backgrounds.

KNOWLEDGE:

Education: Associate's Degree (3) or higher.

Experience: 1-2 years of experience is required (3)

Licensure, Certification: Knowledge of Trauma Informed Care, Mental Health Care, and First Aid are preferred but not required.

Additional Language Required: Bilingual is preferred but not required.

Computers: All positions at IRCO require use of a laptop computer, keyboard, mouse, and cell phone and experience working with Microsoft Products including Microsoft 365, Excel, Word, Outlook, and PowerPoint.
