EDUCATIONAL/TRAINING REQUIREMENTS:Graduate from an accredited college of Pharmacy, PGY1 require, PGY2 SOT preferred
CERTIFICATES, LICENSES, AND REGISTRATION:Licensure to practice Pharmacy in the Commonwealth of Pennsylvania, National Board Certification preferred
EXPERIENCE REQUIREMENTS:Advanced comparable hospital pharmacy experience. Residency, fellowship or equivalent required.Assures optimal drug therapy for the patients by identifying, preventing, and resolving drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient. The clinical pharmacist will provide education and documentation as required by CMS and Transplant Team.
ESSENTIAL FUNCTIONS:
- Assures the appropriateness of drug therapy and compliance with hospital policies.
- Participates is the training of pharmacy residents, students, and interns.
- Assures the implementation of processes to assure the quality of Pharmacy services.
- Interacts with co-workers, visitors, and other staff consistent with the core values of the Hospital.
