By providing case management support, care plans, and follow-up, Nurse Field Supervisors deliver individualized care plans and mentor Home Health Aides to ensure top-quality support. What You Will Do
- Perform initial clinical and ongoing supervisory field visits to patients' homes
- Convey any significant changes in the patient's condition to clinical care team
- Document every patient visit in the EMR
- Orient and supervise Home Health Aides in the field
Requirements
- Current RN license in New York State
- Minimum of two years' experience in home care
- Valid driver's license may be required
