CLINICAL PHARMACIST

The Clinical Pharmacist assumes responsibility for the day to day comprehensive, customer oriented clinical pharmacy services provided by PACE Southeast Michigan, as well as medication utilization and project management under the supervision of the Clinical Pharmacy Manager. The Clinical Pharmacist works in collaboration with the Medical Director and reports to the Clinical Pharmacy Manager of PACE Southeast Michigan. The Clinical Pharmacist must be able to exercise sound management and judgment in making decisions for optimization of drug therapy in elderly participants. In addition, he/she collaborates in PBM oversight, ensuring adherence to Part D regulations and cost-effective medication utilization. He/she assists the PACE Southeast Michigan interdisciplinary team in identifying and resolving drug-related problems for participants. In addition, the Clinical Pharmacist works closely with the corresponding operational pharmacy facility to help ensure accuracy and avoid gaps in care. The Clinical Pharmacist participates in training other health care professionals in the service area. They work closely with medical and administrative leadership as well as primary care providers to develop and implement strategies which continuously improve the quality of participant care at the best value for participants and the organization.

SPECIFIC DUTIES AND FUNCTIONS:

Participant Care

  • Performs participant assessments to facilitate plan of care, treatment, and discharge planning in collaboration with the interdisciplinary team.
  • Reviews and evaluates drug information to identify potential medication therapy problems and recommends appropriate solutions to problems.
  • Evaluates and monitors participants’ response to drug therapy as outlined in the standards of practice, including anti-coagulationand diabetes disease state management via collaborative practice agreement.
  • Communicates participant changes to team members and documents in plan of care.
  • Counsels participants and provides drug information as necessary to caregivers or families about proper use of medicines, possible side effects and adverse effects as needed.
  • Documents all care given to participants in accordance with department standard in a timely fashion.
  • Provides education to other health care professionals, pharmacists and students about optimal drug therapy. This may involve formal preceptor-shipof pharmacy students or pharmacists.
  • Participates, as assigned, in the experiential and didactic education and training of students, residents and support staff.
  • Ensures and promotes customer satisfaction and service.
Comprehensive Medication Review
  • Responsible for comprehensive medication reviews and medication utilization initiatives.
  • Develops and implements policies and procedures to ensure compliance with Medicare Part D regulations.
  • Implements various processes to improve safety and quality, and minimize costs by monitoring, assessing and improving medication utilization.
  • Delegates and directs the activities of support staff providing necessary communication and guidance to those support providers.
  • Participates in monitoring prescribing patterns across multiple sites to ensure pharmacy initiatives and processes are uniformly implemented with appropriate education provided.
  • Reviews current literature, identifies and implements processes to improve appropriate drug use and measure optimal participant care outcomes.
  • Audit records to ensure processes are being followed and provide information to leadership as necessary.
  • Performs other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
  • Registered Pharmacist with the State of Michigan.
  • Controlled Substance license with the State of Michigan.
  • Doctor of Pharmacy degree or equivalent experience with the evidence of advanced clinical training and knowledge in participant care and applied therapeutics, including therapeutic drug monitoring.
  • PGY1 Pharmacy Practice Residency with a focus in ambulatory care or geriatrics preferred.
  • Board Certification in Geriatric or Ambulatory Pharmacy preferred.
  • Must meet a standardized set of competencies before working independently under collaborative practice.
  • Must demonstrate knowledge of managed care and progressive pharmacy practice.
  • Experience in leadership, with demonstrated skills desired.
