We are seeking a compassionate and dedicated Physical Therapist Case Manager to join our team in our residential assisted living homes located throughout the southern metro area. As a PT Case Manager, you will play a vital role in improving the lives of our residents by providing personalized therapy services to enhance mobility, relieve pain, and promote overall well-being.
Physical Therapist Case Manager Responsibilities:
• Assess residents' physical capabilities and develop individualized treatment plans. Ensuring these plans meet requirements of insurance/assistance programs (CADI, Elderly Waiver, etc.).
• Implement therapeutic exercises and activities to improve strength, flexibility, balance, and coordination.
• Provide hands-on therapy and utilize modalities as needed to address specific needs and goals.
• Monitor residents' progress and adjust treatment plans accordingly and ensure all documentation is complete and compliant.
• Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive care.
• Educate residents, their families, and our Care Partners on exercises and techniques for continued progress and independence.
• Participate in the development of the organizational goals, objectives and overall implementation of the program strategic plan.
Physical Therapist Case Manager Qualifications:
• Degree Physical Therapy from an accredited program.
• Valid state licensure as a Physical Therapist.
• CPR certification.
• Prior experience working with seniors preferred.
• Strong interpersonal and communication skills (English).
• Compassionate and patient-centered approach to care.
• Ability to work effectively both independently and as part of a multidisciplinary team.
• Reliable transportation and insurance as daily travel between homes is required.
