**[SUMMARY OF POSITION[: Under general supervision, provides social work and case management services to adult Town residents in need. This may include older adults, families, and adults with various disabilities including physical, psychiatric, cognitive, and substance use disorder(s). [ESSENTIAL JOB FUNCTIONS [:[ Conducts a thorough intake to identify resident needs. Screens residents for appropriate Town, State, Federal, and private programs, and services. Assists with application processes when necessary and appropriate. Makes referrals and linkages to appropriate Town, State, Federal, and private programs, and services. Consults and partners with other town departments around issues of shared concern. **[DESIRABLE KNOWLEDGE, SKILLS, AND ABILITIES:[ Familiar and comfortable with case work methods and techniques. Comfortable working with residents who display a range of personality types and behaviors, including residents with cognitive dysfunction, psychiatric illness, substance use disorder, anger/outbursts and lack of impulse control. Comfortable working with residents with a range of physical conditions and abilities. Proactive when networking and cultivating strong working relationships and partnerships with other Town departments and service providers. Knowledgeable about cultural relativism, intersectionality, structural barriers, and various forms of institutional oppression. [MINIMUM QUALIFICATIONS: [Masters degree in Social Work or a social services/human services related field, such as counseling or psychology. Minimum 1 year experience as a Social Worker or direct social service provider. Internship experience considered.
