Case Facilitator I

A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers.
o Links, monitors, and refers clients to appropriate community resources
o Coordinates care with appropriate internal and external parties
o Ensures appropriate behavioral health services are provided

High School diploma or equivalent.

Experience: Must meet OBHP requirements =Two years' experience in mental health or community services preferred.

Membership: Member in good standing with the agency's Professional Staff Organization at the Case Facilitator level.

First shift, Full time, M-F, no weekends, no holidays.

Salary based on experience.
