POSITION SUMMARY:

This individual performs a dual role as a Medical Case Manager (MCM) and as a program supervisor within the medical case management team at the Immigrant & Refugee Health Center. The supervising MCM will act as the point person on the Case Management team to triage complex MCM issues and to coach other MCMs as well as community health workers. The Supervising MCM assists in program management with the Co-directors and Director of Operations on the technical aspects of the program, and represents the IRHC at hospital and community level events and conferences. The Lead MCM is also responsible for directly supervising staff members of the Immigrant & Refugee Health Center including: hiring, performance management, on-boarding, training, assigning tasks, and discipline.

Position: Lead Medical Case Manager

Schedule: Full Time

ESSENTIAL RESPONSIBILITIES / DUTIES:

Medical Case Management:

  • Conduct comprehensive needs assessment and risk assessment with new patients, including medical, mental health and social needs
  • Provide case management services for established IRHC clients, including connecting patients to a broad range of services including but not limited to: mental health services, primary care services, medical specialty services, immigration legal services, financial assistance, food, transportation, housing, clothing, health literacy, interpreter service needs, etc.
  • Provide client education regarding available services and resources both within the IRHC, throughout BMC and in community, and provide tools and facilitate connections to these resources
  • Assist in facilitation of case management groups for IRHC patients
  • Facilitates interdisciplinary consultation on patient's behalf through participation in rounds, team meetings and clinical reviews
  • Maintains HIPAA standards and confidentiality of protected health information.
  • Performs and maintains all documentation in the electronic chart of patient encounters; resource development efforts; and other patient interventions and interactions
  • Provide consultation to hospital clinics on resources and approaches to care for immigrant patients

Program management:

  • Provides EPIC operational and technical support to case management team members (templates, IT requests and questions).
  • Manage processes and operations for case management team, including: referral and intake processes for case management, refining case management workflows for transportation, allocation of material resources through Patient Care Fund, etc.
  • Support Program Director in creation of Patient Care Fund budget based on case management team needs
  • Track program data in Epic including: referral volume, caseloads, volume of case management visits, encounters, and calls
  • Work with the Program Director to ensure timely and accurate data and metric collection

Personnel Management:

  • Onboard new IRHC case managers, community health workers, and interns with support from administrative coordinator
  • Provide individual supervision, mentorship, and coaching to Case Managers, Community Health Workers, and interns
  • Lead weekly staff meetings for case management team
  • Assign and delegate tasks to case managers, community health workers and interns
  • Ensures staff are meeting program standards (acuity, financial eligibility, ISPs, other DPH requirements)
  • Direct supervision of staff members: including hiring, performance management, onboarding, training, assigning tasks, and discipline.

Program development:

  • Works with the program leadership to create, develop and recommend changes in program model; implements new clinical services, as needed. Ensures culturally appropriate continuum of services for clients.
  • Participate in development of new initiatives, including opportunities for billing for social services
  • Assist in grant writing and applications, QI initiatives and program evaluation processes as applicable
  • Act as a liaison between IRHC and community partners to ensure program can connect clients to a range of services, such as, but not limited to mental health and substance abuse treatment, housing, education and life skills training and employment opportunities.

JOB REQUIREMENTS

EDUCATION:

Masters' degree in social work (MSW) or counseling preferred, or Bachelor's degree in social work or related human services field with minimum 5 years relevant experience.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:

None

EXPERIENCE:

  • 3+ years of experience working with diverse populations
  • Experience supervising staff required
  • Experience and comfort with risk assessment and appropriate triaging required
  • Experience with immigrant and refugee populations preferred, including lived experience as immigrant to the United States.
  • Experience with mental health populations preferred; demonstrated ability to evaluate and appropriately triage mental health crisis situations
  • Comfortable innovating in information technology systems; experience using/documenting in electronic medical record systems
  • Experience in motivational interviewing is a plus
  • Preference for candidates who are bilingual in English and Haitian Creole, Spanish, Portuguese, Somali, Arabic, Dari, or French.
