Community Physical Therapy (CPT) presents an exciting opportunity for individuals passionate about geriatric rehabilitation. We are seeking dedicated professionals to join our team at a picturesque Assisted Living/Independent Living/Memory Care campus in Burr Ridge. The campus comprises a 48-bed skilled nursing facility (SNF) with non-Medicare beds, a 78-resident assisted living building, an 11-resident memory care site, and two independent living campuses. Although the SNF has care plans and meets Medicare requirements, it does not admit Medicare Part A patients.
The role involves delivering outpatient therapy services to residents using EMR-TheraOffice. Visits are required during business hours, potentially Monday to Friday, for 4-5 hours per day, totaling 20-25 hours a week.
