We're dedicated to providing optimal health care to our community, and we're looking for passionate individuals to join our team Position Overview: This is an entry-level professional nursing role focused on promoting wellness and disease prevention in various clinical and community settings. You'll provide primary staff support across programs including Adult Health, Family Planning, Maternal Health, and Infectious Disease Clinics, with a special emphasis on Sexually Transmitted Disease (STD) prevention. Location: Scotland County, NC Schedule: Monday - Friday, 8:00 a.m. - 5:00 p.m. (with occasional extended clinic hours), May be designated as an Essential Employee and required to work during emergencies. Salary Information: Salary depends on direct work related experience; Semi-monthly pay-period. Why Join Us? If you're ready to make a meaningful impact and grow in your nursing career, we want to hear from you Examples of Duties Maternal Health Performs duties staff nurse in the Maternal Health program. Admits patients into the Maternal Health Program. Completes and reviews personal/family, medical, surgical, menstrual, obstetrical, sexual, psychosocial, and dietary self-history. Records significant information about the father of the babys medical history. Record blood pressure, vital signs, weight, and height. Computes EDC (Estimated Date of Confinement) from the last menstrual period. Administers prenatal vitamins and instructions on usage. Checks the admission and completion of follow-up labs and the availability of results are documented and recorded. Counsels patients about healthy pregnancy, warning signs for complications and preterm labor. HIV counseling. Reviews Informed Consent for Maternal Health Services with patients. Refers patient to Maternal Health Provider for physical assessment. Screens return Maternal Health patients. Computes gestational age from best expected date of confinement (EDC). Completes the Maternal Health flow sheet, review record for services and needed labs according to Maternal Health Protocol. Refers patient to Physician Extender or Enhanced Maternal Health Role Nurse to complete prenatal assessment. Assists Physician/Physician Extender with prenatal assessment by scheduling ultrasound or other appointments. Provides counseling and education according to the Maternal Health Education Form. Update Tdap at appropriate time in pregnancy. Administer Tuberculosis Screening. Skin test (PPD) ACCORDING TO Maternal Health Protocol. Offers flu vaccine during flu season. Family Planning Program Screens patients for physical assessment in the Family Planning Program. Measures patients weight body mass index (BMI), temperature, pulse, respiration and blood pressure of each visit. Measures patients height at the initial, annual and/or postpartum visit. Documents the chief complaint, history or present illness, review o systems, age, allergies, last menstrual period (LMP) and last Pap test report on the Family Planning Assessment Form at each patient visit. Updates the patients personal, family and social history at interval visits. Reviews lab work and notifies the physician extender of abnormal findings. Assesses and provides care for patients returning for interval Family Planning services following the physician assistant/physician extender orders and standing orders from the Scotland County Health Departments medical director. Updates immunizations as needed in the Family Planning program. Counsels patient regarding abnormal Pap test findings and plan of care. Schedules a patients return appointment if follow-up can be done within the Family Planning Program. Refers patients to OB/GYN providers if follow-up requires colposcopic evaluation (i.e.- recurrent atypia, dysplasia, cancer-in-situ, invasive carcinoma). Counseling includes informing patients of the Pap test findings and follow-up needed. Enhanced counseling includes pre-colposcopic/biopsy instructions. Special counseling with minors (under 18) and their parent(s) or guardian may be necessary. Evaluates the patients socio-economic resources for OB/GYN referral. Coordinates referral with Carolina Access provider if a patient has Medicaid or personal care physician. Obtains patients permission through a Release of Information form to send abnormal Pap Test report to OB/GYN and Carolina Access provider. Acquire an additional Release of Information form from the patient to receive record of follow-up care from the OB/GYN provider. Send referral form, abnormal Pap test report and Release of Information form for records to providers. Send reminder letters to patients about follow-up in the Family Planning Program and OB/GYN offices. Documents all contacts and attempted contacts with patients in the electronic medical record regarding Pap Test follow-up, counseling and referral. Communicates with OB/GYN providers by phone or mail to check on patients follow-up of abnormal Pap test. Documents follow-up in the electronic medical record notes and problem list. Home visits may be necessary to notify, counsel and refer patients for abnormal pap smear follow-up. May prepare registered letters/correspondences to patients who do not respond or have missed repeated appointments for follow-up. Collaborates with Family Planning Coordinator/Supervisor, physician extenders and nursing staff regarding Pap test follow-up needs for patients. Reviews and updates the Scotland County Health Department Pap Test Guidelines according to Pap Screening Manual A Guide for Health Department and providers from the Division of Public Health and the NC Department of health and Human Services. Assists the physician extender in assessing the patient.
