TGH Hospital at Home is a unique and innovative service that takes inpatient hospital level care to a patient's home. Hospitalist-like patient care is performed in home through a collaboration with the hospital at home attending physician and multidisciplinary team. APP patient care activities include screening for eligible hospital at home patients in TGH's inpatient and emergency room settings. The provider care model primarily consists of daily telehealth visits from the hospital at home command center with an occasional in person visits to a patient's home. Clinical activities include conducting history and physical examinations, patient screening for enrolment, and managing patients medically. APPs exercise autonomous medical decision making (with support from a supervising physician) to provide a broad range of diagnostic and therapeutic services. In addition to patient care responsibilities, APP services should include engagement in quality / process improvement, ongoing program evaluation, and program development for continuous improvement. The acute care Hospital at Home APP will work closely with their Primary Supervising Physician, hospital at home multidisciplinary teams and hospital based consulting teams. Direct reporting is to the Lead APP and the Manager of Advance Practice Medical, Surgical and Neuroscience Services.

Qualifications

QUALIFICATIONS:

  • Nationally board-certified Acute Care NP or Adult Gerontology Acute Care NP, or a Physician Assistant.
  • Inpatient Hospital / Internal Medicine experience preferred.
  • Safe driving record required to operate hospital at home vehicles
  • Excellent communication, critical thinking, and leadership skills.
  • Ability to work effectively in a team.
  • Commitment to ongoing professional development and process improvement.
WORKING CONDITIONS:
  • Work is primarily performed in-hospital via telehealth with use of computer technology for remote patient monitoring and virtual patient visits, in the Hospital at Home Command Center (occasional travel between TGH facilities and to/from patient homes using TGH provided vehicles).
  • Flexible Shifts include: daytime or mid-morning to later evening with occasional weekend coverage.
  • Daily interactions with patients, support team members, physicians, consultants, family members, and occasionally external parties (media, government officials, public).
