The ideal candidate will be results-driven, team-and detail-oriented professional who is comfortable working on multiple tasks in a deadline-oriented environment.
Responsibilities:
- Provide initial assessments and intake evaluations in a clients home
- Provide individual, couples, family or group counseling within the scope of one's competencies.
- Provide crisis intervention.
- Develop and implement treatment plans and document services consistent with Center standards and/or insurance requirements.
- Collaborate with other providers, agencies, insurance companies and individuals in the client's network of care.
- Provide relapse prevention, discharge planning, follow-up and aftercare.
- Provide education, training, and modeling in basic daily living skills, social or parenting skills.
- Teach clients effective techniques for managing their medication as prescribed.
- Coordinate community resources, transportation for client, and referrals to appropriate resources including services within the Center.
- Assist clients in developing and strengthening their support network to accomplish goals and objectives.
- Produce, complete, and maintain billing forms, logs, data collection forms, clinical documentation and client records in accordance with Center standards and state and federal regulations.
- Ensure clients have appropriate medical coverage at all time.
- Accompany clients to community appointment, provides advocacy as needed.
- Schedule and maintain client hours sufficient to achieve individual / program billing targets.
Requirements:
- Master's Degree in the human service field of social work or counseling preferred.
- Minimum (2) year experience working with clients with disabilities preferred.
- Experience or training in related state and federal assistance programs also preferred.
- Licensed at an independent level
- Basic computer literacy skills are necessary
- Valid driver's license and automobile insurance which meets Rhode Island minimum standards law, or the ability to obtain necessary transportation in order to perform the job responsibilities/tasks is necessary for community based positions.
