JOB SUMMARY A registered professional nurse under the direction and supervision of the Manager of 5-South who assists in the daily coordination of staffing and scheduling of nursing personnel for assignment of duties that are consistent with their training and educational experience based on acuity within the Subacute and Long Term Care Units. The Assessment Coordinator will assist with staff development, maintenance of nursing service objectives, standards of nursing practice, identification of geriatric needs in the local community, and assist with the networking and maintenance of relationships with referring case managers and hospital discharge planners. The Assessment Coordinator should also be a resource to administration and participate in the decision making for development of services.
The Assessment Coordinator is responsible for the collection of data used in the screening of potential candidates, the development and implementation of the interdisciplinary care plan and assist with the provision of an ongoing evaluation to monitor effectiveness of services and determines that the established goals are being met within the appropriate time frames, while assisting Memorial Healthcare's mission to meet the health and wellness needs of area people by providing a continuum of personalized programs and services.
JOB SPECIFICATIONS
1. EDUCATION: Graduate from an accredited school of nursing. Current RN license from State ofMichigan.
2. EXPERIENCE: Must have at least two years med-surg experience and to include at least one year geriatric experience; must have working knowledge of current Long Term Care regulations, and preferred to have at least one year's experience in completing the Minimum Data Set (MDS) 2.0.
PRIMARY JOB RESPONSIBILITIES
1. Responsible for the collection of data used in the screening and pre-admission evaluation of potential candidates.
2. Informs Social Services of admission decision.
3. Coordinates admission of new patients.
4. Works with interdisciplinary team to develop and implement plan of care to meet patient's/resident's needs and desired outcomes in accordance with regulatory requirements.
5. Provides an ongoing evaluation to monitor effectiveness of services and determines that the established goals are being met within the appropriate time frames.
6. Assists in the assessment and evaluation of patient's/resident's change of condition, identifies problems, initiates corrective actions, and monitors follow up.
7. Communicates pertinent information in regards to patient to the interdisciplinary team, staff, physician, and family.
8. Develops good working relationships with referring case managers, hospital discharge planner, staff, and other disciplines.
9. Conducts daily review of patient's/resident's condition with the licensed staff for LTC and Subacute Units.
10. Coordinates and participates in team conferences to ensure documentation of patient's progress. Collects written and verbal data from the team to produce a written case summary, status report, and discharge summary.
11. Coordinates clinical services with other patient services to achieve desired patient's outcome.
12. Provides follow up on discharged patients to be shared with team to evaluate the effectiveness of care plan, appropriateness of discharge planning efforts, and customer satisfaction.
13. Demonstrates accountability and skills in decision making, written and oral documentation, and time management.
14. Participates in continuous self development through meetings and inservices as required. Maintains a current knowledge and active interest in contemporary practice, geriatric nursing, trends in nursing, and regulatory requirements for LTC and Subacute.
15. Participates in and supports Subacute Care marketing efforts.
16. Utilizes leadership skills through, but not limited to, participation in staff, physician, and consumer education and training.
17. Demonstrates knowledge of and supports the state nurse practice act code of ethics for nurses and hospital policy and procedures.
18. Keeps patient information confidential.
19. Participates in Quality Assurance activities as assigned.
20. Assists with direct patient care as needed.
21. Demonstrates job related aspects of patient safety to assure a safe environment, safe and effective use of equipment and technology, and decreased risk of potential adverse patient occurrences.
Demonstrates knowledge of and supports hospital mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, customer service standards, and the code of ethical behavior.
