Description
Starting pay: $43.00 per hour
- Explains and coordinates patients' rights and advanced directives; explains Medicare Hospice Benefit and Level of Care Changes when indicated. Understands Local Coverage Determination Guidelines.
- Implements the complete nursing process for assigned patients and their families/representative including; teaching appropriate care, treatment, information and health practices.
- Facilities appropriate admissions and initiates plan of care for each patient which includes identification of nursing diagnosis, identification of goals with appropriate time frames, the design and implementation of appropriate interventions, and the outcome of care rendered.
- Provides hands on patient care, education and teaching, medications, services, and prescribed treatments including those requiring professional nursing skills
- Communicates with the physician, other health team personnel and significant others, and reports changes in the patient's physical and/or emotional condition and patient needs for continuity of care in an effective and professional manner
- Is able to triage and manage a variety of crisis
- Facilitates and supports patient and family self-direction and independence, discharging patients according to agency policies.
- Instructs and supervises the activities of hospice aides/LPN staff.
- Holds in strictest confidence all patient information and discloses information and data only to persons authorized by patient/family/representative or Hospice personnel.
- Demonstrates appropriate infection control measures when dealing with home and inpatient residents.
- Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations.
- Participates in all required in service education programs and meetings.
- Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.

Position Requirements
Required Skills/Abilities:
- Excellent clinical skills to support inpatients and their families at the Home for Hospice. Possess the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of interventions.

- Understands quality improvement programs to help identify opportunities for improvement, problem solving and implementing recommendations.
- Possess strong verbal and written communication skills; demonstrates the ability to communicate effectively with patients, families, and team members regarding care issues.

- Maintains the strictest confidence all patient information and discloses information and data only to persons authorized by patient/family/representative or Hospice personnel.
Education and Experience:
Required Education: Diploma, Associate or Bachelor of Science in Nursing; BSN preferred. Licensed to practice as a Registered Nurse in the State of New Jersey. CPR/AED certification, and Serv-Safe if required.
Required Experience: Minimum one-year of clinical experience in an acute setting.
Preferred Experience: Prior hospice experience preferred.
