Overview

Florida Health Care Plans is seeking a full-time LCSW / LMHC to join our Behavioral Health Department - Crisis Intervention Team in, Volusia County, FL. The ideal candidate will have extensive experience in substance use disorders, "in home" crisis intervention, & the ability to function in high stress situations. The crisis intervention team functions to prevent recurrent hospitalizations of high-risk members and to improve the patient's quality of life through treatment, rehabilitation, and support services. This position will travel between multiple locations including hospitals, community treatment facilities, primary work facility, other FHCP facilities, and member's homes.

Duties and Responsibilities

* Facilitating groups within the department focused on substance abuse, relapse prevention and mental health stability.

* Evaluation, assessment, and treatment of dually diagnosed patients.

* Promotes positive patient relationships.

* Completes required documentation in a timely manner consistent with appropriate documentation standards.

* Coordinates with clinical staff (crisis team, nursing, psychiatry, therapy) in assisting patients and their families in understanding, accepting, and following medical recommendations.

* Performs independently and manages other duties as assigned.

Qualifications

* Florida Licensed Clinical Social worker (LCSW) or Licensed Mental Health Counseling (LMHC)

* Master's degree in social work or other Human Services Field from an accredited university

* 2 years' experience "in home" crisis intervention experience strongly preferred.

* 2 years' experience in substance abuse disorder.

* Certified Addictions Professional (CAP) preferred.

* Experience in working with patients with substance use disorders.

* Current Driver's License

* Computer skills and experience with Microsoft Office programs

* Verbal and written skills necessary to effectively communicate with various members of the health care team, other health facilities, community health related organizations, various external parties, and regulatory agencies
