2023-24 Healthcare Outreach Case Manager

A multidisciplinary team serving individuals experiencing homelessness and chronic health concerns. Delivering Targeted Case Management to support improving access to Social Determinates of Health of those individuals experiencing homelessness whom are also high utilizers of hospital systems. Meet with referrals to the program at St Patrick Center, at partnering hospital, and in the community (local service agencies, shelters, encampments, street). The goal is to improve the health of those experiencing homelessness and decompress the emergency room. This position is a part of Catholic Charities USA's Healthcare Outreach Program, a national intermediary project. As a member, you will meet virtually with other members serving throughout the country, all focused on assisting individuals within healthcare outreach.

Member Duties : This position will speak with H2HH referrals about healthcare goals and how to meet needs outside of hospital utilization. They will provide targeted Case Management including but not limited to applying for Medicaid, establish healthcare providers, ride along with SPC Mobile Outreach Team to locate H2HH referrals that are on the streets, vacant buildings, or encampments. They will assist those members of the community who attend SPC Health Clinic and coordinate services with the partnering agency staff. They will Participate in journaling about the AmeriCorps experience, participate in all trainings, discussion, and meetings arranged by Catholic Charities USA's Healthcare Outreach Program.
