We (Torre) are looking for a dedicated and compassionate Registered Nurse Case Manager to join Physician's Choice Home Health Care in Beaverton, OR on a full-time basis. This role offers competitive compensation and a flexible schedule to support a healthy work-life balance for our team members.
Responsibilities:
- Assess patients' needs to create personalized care plans
- Coordinate patient care with healthcare professionals
- Provide skilled nursing care and monitor patient progress
- Educate patients and families on disease management and self-care
- Advocate for patients' rights and preferences
- Maintain accurate patient documentation
- Collaborate with interdisciplinary teams
- Adhere to regulatory guidelines and company policies
Qualifications:
- Valid RN license in Oregon
- Minimum two years of nursing experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Proficiency in electronic health record systems and basic computer skills
