Position Overview
We are currently seeking a dedicated and compassionate Case Manager | Housing Specialist to join our Housing Program team. In this dual role, you will be responsible for providing strength-based case management to individuals and families facing housing insecurity, while also acting as a Housing Specialist-connecting clients to stable housing through landlord outreach and housing education.
This role focuses on reducing barriers to care such as food insecurity, transportation issues, and homelessness. You will conduct assessments, advocate for clients, and coordinate with community partners to provide wraparound support. As a Housing Specialist, you will also support participants through housing navigation and help increase access to affordable and safe living options.
Key Responsibilities
- Conduct client assessments to identify needs and develop individualized care plans
- Provide case management support including advocacy, referrals, and follow-up services
- Coordinate with community-based organizations and service providers
- Conduct outreach to landlords and property managers to expand housing opportunities
- Educate clients on housing rights, fair housing laws, and the local rental process
- Facilitate housing search workshops and provide 1:1 housing navigation support
- Maintain accurate documentation in the Unite Us platform and case files
- Communicate effectively with external partners and internal teams
- Support client stability and well-being through trauma-informed, culturally competent care
Required
- Bachelor's degree in Psychology, Counseling, Rehabilitation Counseling, or related field
- OR Master's degree in Social Work, Psychology, Counseling, Vocational Rehabilitation, or related field
- Minimum 2 years of experience in case management or related work
- Understanding of intellectual and developmental disabilities
- Strong communication and interpersonal skills
- Technical proficiency with digital platforms (Unite Us training provided)
- Personal or professional experience with mindfulness or meditation
- Familiarity with landlord/tenant rights and fair housing laws
