AltaStaff is a staffing agency currently looking for a Case Manager II to work with our Managed Care Client

We are currently hiring in the state of Virginia, specifically the Roanoke/Christiansburg area

Pay Rate: $32.00 -$40.00 hourly

Shift: Monday - Friday 8am - 5pm EST

This is a hybrid position and field visits will be required

Summary: This role will be responsible for performing case management activities according to the contractual requirements and in alignment with NCQA requirements. This will include potential home visits to complete assessments as well as completion of telephonic assessments, completion of integrated care plans, and conducting interdisciplinary care team meetings. In addition, conducting needed telephonic follow up, education, and care coordination for assigned members.

Responsibilities:

  • Responsible for health care management and coordination of healthcare members in order to achieve optimal clinical, financial and quality of life outcomes.
  • Works with members to create and implement an integrated collaborative plan of care.
  • Coordinates and monitors members' progress and services to ensure consistent cost-effective care that complies with policy and all state and federal regulations and guidelines.
  • Provides case management services to members with chronic or complex conditions
  • Proactively identifies members that may qualify for potential case management services.
  • Conducts assessment of member needs by collecting in-depth information from information system, the member, members' family/caregiver, hospital staff, physicians, and other providers.

POSITION QUALIFICATIONS:
  • Bachelor's degree in Nursing OR Master's degree in Social Work or Health Education
  • LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW or BSW preferred
  • Must have a valid driver's license and reliable vehicle
  • Must have at least one year of experience working directly with the Medicaid population
  • 2-4 years of clinical experience with home care and case management experience
  • Maternity/Pediatrics experience and/or EHR experience preferred
  • Verbal and written communication skills including discussing medical needs with members and interfacing with internal staff/management and external vendors and community resources.
  • Analytical experience including medical data analysis.
  • PC proficiency including Word, Excel, and PowerPoint, database experience and Web based applications.
Work Environment Requirements:
  • Must be able to to lift at least 30lbs
  • Must be able to stand or sit for prolonged periods (75% of the time)
  • Private Secured Internet Connection
  • Must confirm work environment is safe and secure for work in handling private and sensitive information.
  • Constantly operates a computer and other office equipment
  • Constantly works indoors in a secluded safe space
  • Must have High-speed internet access 25 MBPS minimum; 50 MBPS minimum preferred
