_Occupational Therapist - Home Health Care_
Join the team Now accepting applications for a Full-Time Occupational Therapist in Warren County and surrounding areas. Darling's Home Care, based in Warren, offers a full range of skilled services including skilled nursing, physical therapy, occupational therapy, speech therapy and medical social services. 24/7 on call nursing services.
Responsibilities of an Occupational Therapist:
Coordinate the total Plan of Care for the patient in the Home Care setting assessing, planning, implementing and evaluating.
Organize daily assignments, setting priorities based on individual and overall needs of the clients while traveling to each home as scheduled.
Provide leadership for the team by working cooperatively with other Darling's Home Care disciplines, skilled nursing, physical therapy, speech therapy and medical social services.
Educate the patient/caregiver in the management of their illness and promote the home health support systems to decrease re-hospitalization and utilization of hospital emergency services.
Balance your clinical talent with compassion and focus while meeting the needs of multiple patients throughout your day.
Qualifications:
Home Health Care experience preferred but not necessary
Licensed with current PA state license in good standing
Self-directed with excellent observational ability and effective communication skills
Ability to provide quality written documentation in a timely manner
Valid driver's license with current auto insurance
Ability to work independently and within a multidisciplinary team
