JOB PURPOSE/SUMMARY

Summary of role of team:
  • The Intensive Long-Term Support Program (ILS) is classified as an "adult supportive residential facility" providing 24-hour residential care incorporating treatment and rehabilitation components.
  • Services are provided to include the following: psychiatric services, nursing services, case management services, wrap-around and recovery services, transportation services and personal care services.
  • The ILS Program promotes training in community living skills and increased socialization opportunities through the provision of daily leisure and psycho-educational activities.
  • The program is staffed with a minimum of one staff member for every eight service recipients 24 hours per day/seven days per week/365 days per year.
  • The program provides services to adults who are age 18 or over and have received inpatient psychiatric treatment for a mental illness or co-occurring disorder.
  • Service recipients must have been discharged from one of the state RMHI's or one of the state contracted providers (SCP's) of inpatient psychiatric services to uninsured adults.
Summary of position:
  • Provides direct care to clients residing in an Intensive Long-Term Support (ILS) program, while also providing direct supervision to other nursing staff and milieu staff.
  • Responsible for ensuring that each client's mental health and medical health needs are met via the completion of medical assessments, administration of medications, collection of vital signs, etc.
  • Must be able to work flexible hours based on the needs of the program.
  • Mandatory to remain awake and alert during shift to monitor client safety.
  • Must be able to provide transportation as needed.
TYPICAL WORKING CONDITIONS/ENVIRONMENT
  • Position is based within the ILS Program, which is an Adult Supportive Residential facility that provides services 24/7.
  • The role of the ILS RN Team Leader is to ensure that coverage is provided 24/7 despite any weather conditions that may arise.
  • This position requires frequent community involvement and group community outings.
JOB DUTIES/RESPONSIBILITIES

1. Works in a multi-disciplinary team approach to meet clinical needs.
  • Attends treatment team on a weekly basis without tardiness & participates.
  • Responds to all flags, emails, and voicemails within 1-2 business days.
  • Returns calls/pages within 15 minutes.
  • Takes a lead role in hiring and retaining staff. Maintains appropriate level of staffing.
  • Assists with administering medications and providing staff coverage as needed.
  • Assists clients with completion of self-care / hygiene tasks.
  • Assists clients with maintenance of their person as necessary.
  • Effectively implements the Re-Ed behavior management system each shift.
  • Clearly communicates to team members throughout shift regarding client behaviors, needs, and any other relevant information.
  • Works in a multi-disciplinary team to meet the needs of the SPMI population and oversees clinical standards.
2. Completes all documentation in compliance with CARF and funder standards.
  • Properly documents Re-Ed related information in the client communication log and point log before the end of each shift.
  • 15% chart sample evidences that individual treatment plans are signed by the RN every 3 months.
  • 15% chart sample evidences that AIMS are updated every 3 months.
  • 15% chart sample evidences that allergies are updated monthly.
  • 15% chart sample evidences that education was completed with each client at individual appointment.
  • 15% chart sample evidences that all med consents are up to date for all medications.
  • 15% chart sample evidences that all notes are completed within 48 hours of contact.
  • 15% chart sample evidences that all notes are detailed and contain all necessary documentation.
  • 15% chart sample evidences that prior authorizations are completed within 2 business days and appropriate documentation to evidence this is in clients chart.
3. Provides linkage and/or referral to primary care physicians for treatment of medical needs.
  • Will complete a medical assessment on each client 2 times per week.
  • Assists with returning / resolving primary care physician and all other medical provider calls as needed.
  • Ensures that clients medical needs are met; monitors and assists with referral to PCP/ER as appropriate for medical concerns.
  • Ensures that clients maintain a supply of all meds prescribed by outside practitioners.
  • Ensures that vital signs are obtained 2 times per week and PRN.
4. Demonstrates and maintains a positive work climate and the overall team effort of the department.
  • Demonstrates a consideration and concern for fellow workers and their jobs and promotes harmonious relationships and attitudes.
  • Accepts additional assignments and / or changes in assignment and/or work duties.
  • Promotes an environment in which the customs and spiritual beliefs of the individual are respected.
  • Meets with staff on a consistent basis to address identified concerns.
  • Gives and receives appropriate feedback.
  • Communicates directly and approaches conflict with a problem-solving approach.
  • Address staffing issues or problems with the Program Coordinator within 2 business days of the event.
  • Organizes and develops team cohesion.
5. Ensures that program productivity targets are met or exceeded.
  • Ensure that clients maintain a supply of all internally prescribed medications.
  • Ensure that clients receive all medications prescribed as indicated on the MAR.
  • Takes a lead role in hiring and retaining staff. Maintains an appropriate level of staff (covers shifts as necessary for staff absences.
  • Fill vacancies within 2 weeks previous employee's termination.
  • Trains staff regarding program P&P within one week of completing orientation.
  • Ensures each supervised LPN completes documentation before the end of their shift.
  • Reviews all LPN documentation for accuracy and completeness.
  • Ensures each supervised LPN completes required CARF modules and other training materials.
6. Completes a variety of reporting .
  • Ensures that the Meal Log is completed each day and monitors food/fluid intake of each client. Discusses concerns in treatment team weekly.
  • Monitors Relias on a monthly basis to ensure that staff members remain up to date on required trainings.
  • Submit SAL to Program Coordinator for previous weeks contacts every Saturday.
  • Create and submit monthly health activity curriculum calendar to Activities Coordinator by the 15 th of each month.
  • Submits all expense requests for clinical supplies within 48 hours of making the request.
7. Effectively networks with internal and external staff members.
  • Maintains positive relationships with internal departments to meet program needs and maximize growth.
  • Develops a positive and working relationship with outside agencies to meet program needs.
  • Gives and receives appropriate feedback.
8. Works to maintain a positive public image .
  • Ensure that ILS residents follow program rules and expectations during community outings.
  • Model appropriate social behaviors and provide individual and/or group opportunities to enhance/develop appropriate social behavior.
  • Leave other facilities in a clean and organized manner when groups are concluded.
COMPENSATION:
  • Starting salary for this position is approximately $32.01 /hr based on relevant experience and education.
Schedule:
  • Flexible to meet the needs of 24/7 program.
  • Will provide coverage for staff supervised when needed.
  • Must be available to be on-call for ILS needs.
Travel:
  • This position requires utilizing a personal dependable vehicle to conduct Center business.
  • Maintaining a dependable vehicle and certified driver status is a condition of employment.
  • F Endorsement is required for this position and must be able to operate company owned vehicle as well.
Equipment/Technical Competency:
  • Must be efficient in computer skills.
  • Use of Microsoft Office is required.
  • Must be able to use a fax machine and phone system.
  • Computer skills and ability to utilize company program software to properly document.
QUALIFICATIONS - ILS RN Team Leader

Education/Knowledge:
  • Must have an RN degree with a current Tennessee license; knowledge base in a health-related field/nursing field. In addition, course work or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and mental retardation.
  • RN degree preferred.
  • This position requires utilizing a personal dependable vehicle to conduct Center business.
  • Maintaining a dependable vehicle and certified driver status is a condition of employment.
Experience / Knowledge:
  • 1-year experience required; experience with SPMI population preferred.
  • 1-year supervisory experience is preferable.
Physical/Emotional/Social - Skills/Abilities:
  • Exposure to biological hazards.
  • Hearing of normal and soft tones.
  • Close eye work.
  • Valid driver's license- F endorsement.
  • Lifting up to 50 lbs.
  • Pushing/pulling up to 150 lbs.
  • This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking. (training provided)
  • Frequent sitting, standing, walking, bending, stooping, and reaching.
  • CPR and First Aid certification required. (training provided)
