Job description: Join our healthcare client, a rehabilitation facility providing post-acute care located in Antioch, CA:
8 OR 13 week contracts
Schedule: Sunday - Thursday OR Tuesday - Saturday
Job Description:
Develop and obtain approval for treatment plans from referring physician.
Treat patients according to physician-prescribed plans.
Assist nursing department in Restorative Aide training.
Supervise Physical Therapy Assistants in patient care, adhering to state regulations.
Communicate patient progress and plans with team.
Attend Patient Care Conferences and Rehabilitation Conferences as needed.
Participate in staff in-services training.
Document evaluations, progress notes, and recertification in compliance with PT Board regulations.
Record treatment changes following policies.
Provide family/caregiver training for discharge.
Assist in discharge planning.
Recommend and facilitate ordering of durable medical equipment for patient mobility.
Report equipment issues promptly.
Foster positive relationships with patients, families, and colleagues.
