CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services. About Community Physical Therapy (CPT):
Community Physical Therapy (CPT) is a leading provider of rehabilitation services across the greater Chicagoland and southern Wisconsin areas. We seek Occupational Therapists for PRN positions at multiple Alden Estates locations. Our board-certified therapists provide Physical, Occupational, and Speech Therapies within post-acute rehabilitation, skilled nursing facilities, home settings, and outpatient locations.
CPT Services Include:
Rehabilitation services in skilled nursing facilities
Memory care
Assisted and Independent Living
Outpatient therapy

Job Responsibilities:
Adhere to CPT and facility policies and procedures in delivering therapy services.
Maintain the highest standards in rehabilitation services.
Perform administrative duties to support the daily operations of the rehab department.
Build strong rapport with patients, families, third-party payers, and others involved in patient care management.
Qualifications:
Graduate of an accredited Occupational Therapy program approved by the American Occupational Therapy Association (AOTA).
Licensed, certified, or registered to practice as an Occupational Therapist in Illinois or Wisconsin.
Certified by the National Board for Certification in Occupational Therapy (NBCOT).
Adheres to the AOTA Code of Ethics and Standards of Practice.
Preferred Experience:
Skilled Nursing Facility (SNF) experience is preferred.
Knowledge of Net Health is a plus, though training is available.
New graduates are welcome to apply.
