*Summary*: The Licensed Clinical Social Worker (LCSW) functions as a member of an inter-disciplinary team and provides services to patients and caregivers that are necessary to address social and emotional problems that are or are expected to be an impediment to the effective treatment of the patient's medical condition or rate of recovery. Services provided may include but are not limited to, bio-psychosocial assessment and evaluation of factors impacting the patient's ability to participate optimally in the treatment plan, providing education and linkage to additional financial and community resources, advocacy, and counseling services to the patient and short-term counseling to caregivers. The LCSW serves as patient advocate and liaison between other healthcare entities, facilities and community providers, resources and agencies, working collaboratively to ensure seamless transitions across the continuum of care. The LCSW confers and collaborates with physicians in implementing the treatment plan.
