Nurse Practitioner

HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with a Fortune 100 major Healthcare organizationin Portland, Oregon.

Daily Responsibilities for Nurse Practitioner:

  • Conduct comprehensive assessments, patient facing
  • Effectively manage acute and chronic medical and behavioral conditions
  • Ensure accurate and complete documentation, including ICD 10 conditions.
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education
  • Schedule: Monday-Friday 8:00am - 5:00pm

Travel questions
Where they will travel: Portland, OR
Counties: Multnomah and Washington
When will they travel: During Shift
How many facilities will they be traveling between: 4 facilities and private homes

Required Qualifications for Nurse Practitioner:
  • Active and unrestricted license in the state which you reside, or ability to obtain by start date
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Minimum 1 year NP experience
  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
Family Nurse Practitioner
Adult Nurse Practitioner
Gerontology Nurse Practitioner
Adult-Gerontology Acute Care Nurse Practitioner
  • Covid vaccination
