About the Role: Nurse Practitioner - In-Home care Embrace the chance to redefine your work-life balance with Easy Health's innovative and flexible approach to healthcare. As a Nurse Practitioner at Easy Health, you will be at the forefront of delivering preventive care and educational services, directly engaging with members in the heart of their communities. This position offers the unique fulfillment of local travel and the opportunity of face-to-face interactions with members directly in the comfort of their homes. You'll play a vital role in performing comprehensive assessments such as Annual Wellness & Quality Gap Care visits for our members in their homes. Location: We will be hiring in Virginia (Arlington and Alexandria) and would love the opportunity to speak with you. The applicants must be local and have reliable transportation for travel to at least one of these areas. Schedule: 24-40 hours of availability, Monday - Friday 8:00 am to 5 pm Seeing between 6-8 patients per day. Timeframe per visit: 45 min to 1 hr. sessions. Compensation: Starting from $120 per completed visit Payment for same day no show visits Independent Contractor Reimbursement for travel mileage What You'll Do: Perform clinical screenings and various tests in an in-home setting. Accurately code, verify, and document clinical findings. Complete a detailed evaluation of the medical and social assessment of the beneficiary members using the company's proprietary tool or client EMR. Identify and refer members/patients for routine, urgent, and emergent case management. Make recommendations for member's follow-up with their primary care physician. Note: Not required to prescribe or alter the member's current treatment plans. Privacy - maintain a comprehensive understanding related to protected health information and observe HIPAA requirements. Professionalism - maintain warm, caring, and courteous behavior with the members. Member Satisfaction - make sure that the member is satisfied with the exam and overall visit. Member Communication - maintain member privacy and courteous communication with members and colleagues. Safety - maintain a safe working environment. Follow the safety policy and procedures listed subsequently. Who You Are: Our ideal team member is driven by a profound dedication to providing top-tier care. Other qualities include: 2 years of hands-on experience as a Nurse Practitioner Current, unrestricted license in the state(s) in which the services are rendered Board-certified, preferably geriatric or adult medicine specialty Familiarity with the HCC model and quality HEDIS and other STAR measures Effectively communicate with elderly and chronically ill members Proficient in the use of computer, tablet, and mobile technology Willingness to travel within the geographic area.
