As part of our mission to bring personalized, quality-based healthcare to patients at home, the Medical Social Worker plays a crucial role in understanding and addressing the social and emotional factors related to their health problems.

Responsibilities

  • Conduct social casework and preparation of social histories to determine service needs
  • Develop and maintain treatment plans, including discharge planning
  • Coordinate referrals to community agencies
  • Participate in team conferences to identify social problems and assess family circumstances

Qualifications

  • Current Social Worker License
  • Master's Degree in Social Work
  • One year of social work experience in a health care setting
  • Current driver's license, good driving record, and reliable mode of transportation
  • Able to make sound professional clinical judgment
  • Able to assess and document patient needs
  • Proficiency in Microsoft Office
