Coler offers long term and subacute care, rehabilitation, and a dedicated memory care unit, comprised of 150 beds featuring an acclaimed music and memory program. The facility features a robust rehabilitation therapy program with expansive gym space for residents. Coler also offers full time physician staff, psychiatry, dental services, an onsite 8-chair dialysis den for hemodialysis, and on-site medical clinics to meet the clinical needs of its residents.

Job Description

MAJOR TASKS|RESPONSIBILITIES:
• Plans, organizes and implements Therapeutic Recreation programs based on the assessed need and interest of resident on assigned units, reflective of culture, leisure history, current interest and present abilities/limitation.
• Assesses the problems, needs and interests of every patient/resident and completes assessments on assigned units upon admission and annually.
• Develops treatment plans for every patient/resident based on assessment.
• Develop treatment plans for every resident based on assessment. Complete the care planning including completing MDS.3.0 forms and documents quarterly progress notes or as needed.
• Conducts age appropriate, Therapeutic Recreation programs on assigned units addressing needs and interest of residents.
• Documents residents progress in charts on a quarterly basis or as needed in progress notes & care plans.
• Develops, implements a monthly schedule of therapeutic and leisure programs on assigned units and centrally.
• Submits monthly food, funding and other needs for unit trips, unit birthday parties etc. Coordinates and or assist in the planning and implementation of special events.
• Attends departmental meetings and in services, may be asked to conduct training sessions.
• Assists in the planning and implementation of special events, trips, unit birthday parties, etc.
• Maintains monthly bulletin board and calendars reflecting available therapeutic and leisure activities.
• Maintains and orders supplies and equipment necessary for TR programs.
• Maintains and input dail
• basis program participation statistics for unit/central programs and special events.
• Submits all required departmental request, e.g., petty cash request, out trip referrals and requests, special food request/standing order on a timely basis.
• Conducts recreation programs either on assigned units or centrally on a daily basis, including evenings and weekends.
• May provide supervision and evaluation to Activity Therapists, Student Interns, or volunteers assigned to assist in their programs.
• Participates in Interdisciplinary Care Plan meetings or Committee involving direct patient care, completing necessary documentation ex. MDS 3.0 OR CCP's etc.
• May assist in Performance Improvement Activities.
• Conducts monthly Community Planning Meetings with Residents and staff to include their on-going ideas of programming and neighborhood concerns and improvement.
• May be asked to contribute to the development of policies, procedures and protocols for the Department through committee work chaired by a Manager or Activity Therapist III.
• Follow all facility P & P for safety management, pain management and infection control.
• Participates in interdisciplinary PI.
• May supervise college for interns pursuing future credentials.
• Follow facility policy regarding Outlook e-mail account. Check account daily.
• Escort to and from program as needed.

TUE - SAT 9AM - 5PM

Minimum Qualifications

Assignment Level I 1. A baccalaureate degree from an accredited college including or supplemented by 18 credits in recreation, physical education, dance, fine arts, applied arts (including therapy) and 12 credits in psychology, social psychology, group work, group dynamics and/or related field; or
2. A baccalaureate degree from an accredited college and one year of full-time experience in planning, coordinating and supervising a recreation activities program in a hospital, health care or related facility; or 3. A satisfactory combination of education and/or experience equivalent to 1 or 2 above. However, all candidates must possess a baccalaureate degree.
Assignment Level II
In addition to meeting the requirements for Assignment Level I, Assignment Level II requires:
1. One year of full-time experience as described in two (2) above.
Assignment Level III
In addition to meeting the requirements for Assignment Levels I, Assignment Level III requires:
1. Three years of full-time experience as described in two (2) above.

Department Preferences

CTRS (Certified Therapeutic Recreation Specialist

CDP (Certified Dementia Practitioner)

Knowledge of Recreation Therapy practices, methods and techniques.

Strong interpersonal skills.
