Your Role with Us:
Vitability Health of New Jersey invites you to bring your clinical prowess and care management expertise to a role that goes beyond traditional nursing. As our Healthcare Liaison, you'll be the linchpin in crafting smooth transitions for patients from rehab to home, working closely with a multidisciplinary team to tailor post-discharge plans. But that's not all. We're looking for a dynamic individual who can also wear the hat of a business developer - someone who can nurture and grow our partnerships with healthcare facilities through strategic relationship-building and effective communication.

  • You have a Valid LPN or RN license in New Jersey and 3+ years of experience in a nursing facility or similar healthcare environment.

What You'll Do:
  • Conduct comprehensive patient assessments, working with healthcare teams to ensure every patient has a custom post-discharge plan.
  • Be the essential communication link that brings together Vitability Health New jersey and our healthcare facility partners, ensuring clarity and coordination in care plans.
  • Identify and coordinate the arrangement of patient support services, using your care management expertise to address post-discharge needs.
  • Employ your flair for business development to build and deepen professional relationships with healthcare facility partners, contributing to our mission of expanding patient-centered care solutions.
