Hospice RN Case Manager

We are looking for a compassionate and empathetic Hospice RN Case Manager to join our team. In this role, you will have the opportunity to provide direct patient care services to hospice patients and offer supportive care to their families. As a key member of the interdisciplinary team, you will play a crucial role in ensuring that each patient receives the highest quality of care and support.

Responsibilities:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior
  • Act as a primary liaison between physician, patient/family, and hospice team
  • Empower patient's caregivers through education on care for their loved ones
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs

Requirements:

  • RN license in the state of requested employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
  • Current driver's license and automobile insurance
  • Current CPR certification
