Mountain Valley Hospice & Palliative Care is dedicated to providing exceptional end-of-life care that aligns with our mission of compassion, dignity, and support. We are seeking a dynamic and compassionate Hospice Liaison / Admission RN to join our team in Danville, VA working Monday – Friday, 8a – 5p. This role is crucial in bridging the gap between healthcare providers, patients, and their families, ensuring a seamless transition into hospice care.
Key Responsibilities:
- Collaborative Care: Partner with the growth and access teams to implement the organization's long-term goals, delivering patient and family care services that reflect our mission of compassionate care.
- Relationship Building: Establish and maintain relationships with physicians, hospitals, health facilities, and community resources to enable effective and efficient referrals to our services.
- Patient & Family Engagement: Meet with patients and family members to discuss and assess hospice eligibility, emphasizing reducing anxiety and assisting with the transition. Explain the hospice admission process, including Patient's Rights and Responsibilities.
- Health Information Management: Receive patient health information from physicians, hospitals, clinics, and other agencies. Document patient information and all orders/instructions for care, and submit to physicians for certification of terminal illness.
- Clinical Collaboration: Proactively collaborate with clinical staff to ensure the organization can meet client needs before admission.
- Referral & Admission Coordination: Initiate the referral intake process and coordinate admissions with appropriate team members and facility staff when applicable.
- Comprehensive Assessments: Perform patient start-of-care comprehensive assessment, evaluation, development, and implementation of plan of care.
- Skilled Nursing Care: Provide skilled nursing care to HIP patients in accordance with the plan of care.
- Valid RN license in the state of Virginia.
