Nursing Care Coordinator Sign-On Bonus Available

Department: Utilization Management

FTE: 0.75 (Part Time)

Shift: Days

Job Summary:

  • Coordinate the care team approach for quality, cost-effective treatment of patients.
  • Manage patients' treatment, collaborating with other staff and ensuring efficient resource utilization.
  • Conduct initial and ongoing patient assessments, and implement disease management protocols.
  • Determine and manage patient outcomes.
  • Function as a point of contact for patients, family members, medical teams, and community resources.
  • Ensure adherence to hospital and departmental policies and procedures.
  • Patient care duties may involve neonate to geriatric age groups.

Responsibilities:

  • Identify patients in need of nursing interventions.
  • Assess clinical and psychosocial patient status.
  • Provide comprehensive care plans and treatment options, discharge planning, and use of appropriate resources.
  • Coordinate referrals to home health, home infusion or equipment services, as required.
  • Maintain computer-based records.
  • Develop relationships with community healthcare agencies and act as a resource on complex patient care activities.
  • Assist in the development of patient-centered goals for individuals and families.
  • Collaborate with healthcare teams to ensure continuity of care across all settings.
  • Provide and document patient/family education specific to identified needs.
  • Participate in team meetings and attend Hospital related committees or the community as required.
  • Complete continuing education requirements, enhance professional growth and development.

Qualifications:

  • Graduate of a nationally accredited nursing program.
  • 1 year of directly related care experience.
  • Current RN licensure in New Mexico.
  • CPR certification for healthcare/BLS Provider or Professional Rescuers within 30 days of hire.
  • Physical ability to perform Light Work.
  • Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni or Navajo preferred.
