Assesses and documents the needs and concerns of patient and family to enable individualized care. Supports access to services and programs that constitute the patient's journey by obtaining answers to questions and concerns. Provides appropriate written and oral explanations and information. Facilitates communication between the patient and healthcare professionals and promotes timely delivery of services. Explains the cancer care process. Supports patients in the decision-making process. Assesses patients prior to their procedures by studying patient medical histories and speaking with both patients and their physicians. Helps patients better understand the procedures they need and serving as a liaison between the patient, nursing staff, and physician. Takes vital before and during procedures. Reviews and screens medical record for blood thinners and working and Anticoagulation department as needed. Demonstrates knowledge of commonly used meds: action, dose, and side effects. Documents procedure notes in the electronic systems. Makes or facilitates referrals to volunteer and community programs. Assesses the emotional needs of patients and their families. Refers patients and/or families to supportive/rehabilitative care services, both professional and volunteer. Promotes patient's reinvestment in activities of daily living following treatment. Provides guidance to resources to support families during bereavement. Acts as a liaison between healthcare professionals and the family. Provides evaluative feedback to OMC as it impacts the patient's cancer care experience. Educates patients and families concerning what to expect during the cancer journey. This will be done through provision of resources, either oral or written, including recommended websites. Refers health professionals to appropriate educational resources and services. Documents procedures, work-flows, and policies. Other duties as assigned. This position provides nursing coverage for the Breast Center and the department of radiology. The nurse supports patients and their families in contact with the services they require at any point in the cancer experience, from abnormal mammography, through to treatment and after care. This provision of service will improve patient satisfaction with the healthcare system associated with the cancer experience. There will be a reduction in the anxiety and frustrations currently experienced. Healthcare providers will also benefit from the coordination of services and information provided by the radiology nurse. They will bring about more efficient and cost-effective use of healthcare resources. The other aspect of this position is the supporting of all the patients in the radiology department. The medical imaging nurse will receive referra s from OMC clinic programs. Their response will be to facilitate access to information, support, and services appropriate to the needs of women with breast cancer and their families. The medical imaging nurse will assess the current system, make recommendations for changes, assist in the development of an improved system, and evaluate the outcomes. They will work with breast cancer patients, their families, and members of the healthcare system to: provide an explanation of the cancer care system to patients and their families assess the needs of patients and families to enable the most appropriate provision of services in a timely and cost-effective manner enable patients to be part of the treatment process to the degree they desire provide a smoother journey for the patient and their family through the cancer system by facilitating the coordination of services improve communication between health professionals to ensure efficient and cost-effective delivery of services and resources. The medical imaging nurse will act as a patient advocate and facilitate communication between the patient, family, and healthcare professionals. This consistent and continuing communication and support will facilitate a holistic approach to planning, organizing, teaching, and relationship development to better meet the needs of the patient.
